MMU Application Guidelines

We are excited to receive your application! To increase your chances of acceptance to the program, please keep the following in mind.

What a qualified applicant looks like

We look at these three primary qualifications in MMU applicants.

Application Materials

Each of the following application materials must be completed to move on to the next step in the application process, the phone interview. Each of these application materials are also taken in to consideration when making acceptance decisions. Please take your time and review your application materials before submitting to better your chance of moving on to a phone interview and being accepted to the program.

All application materials aside from your online application should be submitted to mmu@midlevelu.com.

Online Application Form

Each applicant must complete the online application form (below). The more information you share with us, the easier it is for us to match you with your ideal location.

Resume

Your resume will be used in the MMU matching process. We highly recommend modeling your resume after this sample to get your first impression with sites off to a good start.  You can download the sample resume here.

A professional looking resume not only lets us know you are serious about applying,

it also lets your potential sites know you are serious about your career.

Recommendations

MMU requires two recommendations. These must be completed by one professional and one academic reference or two professional references. Choose wisely as we take the recommendations heavily into consideration as we make acceptance decisions.

Transcripts

We need official graduate program transcripts. Haven’t graduated? No problem, send us your unofficial transcripts for now and we can get your official copy after you graduate.

Personal Statement

This is an opportunity to communicate to MidlevelU your qualifications, interest and motivation for applying to the Midlevels for the Medically Underserved program. Each question should be answered honestly and completely. We do take grammar and content in to consideration when making acceptance decisions, so take your time. Please download the personal statement questions here and submit your written responses to mmu@midlevelu.com.

We want to make sure you will represent yourself well in the employment setting. We pay close attention to your resume, references and phone interview.

1.  Professionalism

2.  Academic Achievement

We look at your GPA and essays. We don't have a strict GPA cutoff but qualified applicants have a strong record of academic success.

3.  Career plan/goals

We want to make sure that your career goals align with the program. This means that you’re interested in primary care, at least as a start to your NP career. We also want to know that you'll be a good fit for working with underserved patients.
